Survey helps us serve our communities’ needs
Every three years, Asante conducts a community health needs assessment (CHNA) to help identify issues that impact the health of the people who live in our local communities.
We’d like to hear from you! Take the online survey today. You can join other Southern Oregon community members for a chance to win one of several $50 gift cards.
The survey asks basic questions about your health, what makes being healthy easy or hard, and how well our community supports healthy living through various resources.
Your responses, along with hundreds more from our community members, will help us direct our own energy and resources toward meeting the most critical health needs.
The survey is open now through Sunday, March 20.
About the community health needs assessment
The IRS encourages hospitals to collaborate with other health care providers within the region to conduct jointly sponsored CHNAs. As such, Asante is partnering with Providence Health & Services, Southern Oregon Region, for our 2022 assessment.
Community members throughout Jackson and Josephine counties, as well as some in our secondary service areas, will participate via listening sessions, key informant interviews, and online and paper surveys.
Results of the assessment will be compiled into a final report that will be shared throughout our community. It will provide a snapshot of the challenges and opportunities related to the health needs of people living and working within our service area.
Asante will use results from the CHNA as we develop strategies for each Asante hospital that will help address the prioritized health needs.
