If you and your family members are enrolled in an Asante Health Plan option and need to establish care with a new primary care provider with Asante Physician Partners, Asante has a resource to help you find one that will meet your needs.
The New-Patient Access team is available to connect you with providers who are accepting new patients. The team is available Monday through Friday from 8:30 a.m. to 4:30 p.m. at (541) 789-1234.
As a reminder, when you use an APP provider your coverage falls under the Category 1 tier, so you will save the most in your out-of-pocket expenses.
