Employees and dependents enrolled in the Asante Health Plan have access to free support, tools and devices from Livongo, a digital service that helps with weight management, hypertension and diabetes.
Here’s what you get when you sign up with Livongo:
- Advanced tracking technology. Based on the program chosen, you can receive equipment, such as a free smart scale, a connected diabetes smart monitor or a blood pressure monitor. If signed up for diabetes management, you can also receive unlimited test strips and lancets at no cost. If your glucose levels go out of range, Livongo will reach out to you within 15 minutes to get you the support you need.
- Expert coaching and guidance. You can find personal meal and nutrition plans, summary reports, support from coaches and more.
