Diabetes Education and Nutrition Services restructure

As Asante continues to evolve, we are identifying opportunities to be innovative and improve our service distribution. In some cases this means scaling back our services; in others, expanding them.

One service we are currently restructuring is our Diabetes Education and Nutrition Services program offered virtually through Asante Physician Partners in Medford and Grants Pass.

Beginning Nov. 4, the program will care only for patients who are enrolled in the Asante Health Plan, AHNPlus, or whose provider is aligned with Asante through the Asante Health Network.

This program became a virtual service in 2020 when the pandemic hit. It has since transitioned to both in-person and virtual, serving about 100 patients each week. The goal moving forward is to provide education alongside primary care providers.

“These and other changes reflect the fluctuating times in health care,” said Jamie Grebosky, MD, chief medical officer. “Asante will continue to assess and adopt innovative models of care and reallocate resources to provide the best care to our patients.”

Of the 13 employees in the department, some team members will continue to provide diabetes education, while the other employees will explore opportunities within the health system.

In the coming months, we will share additional announcements about other planned changes, including an expansion of women’s services in Ashland and APP–Maternal Fetal Medicine clinic’s move to bigger digs in Medford.
