Nov. 15: “The Four M’s of an Age Friendly Health System”

As Asante Ashland Community Hospital works toward becoming a certified Age Friendly Health System, clinicians have an opportunity to gain insight and skills in caring for older patients.

In the first of a series on age-friendly care, geriatrician Candace Kim, MD, discusses the four M’s of inpatient geriatric care: Medications, Mobility, Mentation and what Matters. The hour-long online presentation takes place at 7 a.m. on Tuesday, Nov. 15.

An Age Friendly Health System is an evidence-based program supported by the Institute for Healthcare Improvement. These grand rounds are made possible with the support of the Asante Foundation’s Skyrman Educational Fund.

Candace J. Kim, MD, is a clinical associate professor of medicine in the Division of Geriatric Medicine at University of California, San Francisco, Medical Center. She attends on the UCSF Geriatrics-Orthopedic Surgery Co-management Service and the Acute Care for Elders (ACE) services at UCSF Parnassus and Zuckerberg San Francisco General Hospitals.

Dr. Kim has partnered with the Zuckerberg San Francisco General Hospital Emergency Department to create a multidisciplinary Age Friendly Emergency Department.

Her academic interests include optimizing emergency and acute care for older adults, educating residents and fellows on post-acute care transitions, and improving outcomes after surgery through a geriatrics co-management model of care.
