Adverse event reporting made Symplr

Report your safety concern to the Asante Risk Management channel on the former Halo app.

As part of Asante’s Safety First initiative, we’re making it even easier to report serious safety concerns. Risk Management has created a new messaging channel within Symplr (formerly Halo) which allows for on-the-spot reporting.

The channel is called “Asante Risk Management” and will be monitored for swift review and resolution.

Staff may use this group to report any adverse event in which a patient was harmed as a result of their care, ranging from serious harm to death. Adverse events also include incidents that pose an imminent threat or have led to the harm of our staff.

It’s important to note that the Symplr reporting channel does not replace submitting a Responsible Event Report. RERs should be completed by any staff member who witnesses or hears of an event such as those listed above so the incident can be further investigated.

Our goal is to have staff use the Symplr tool to communicate concerns that might require immediate action. For nonurgent communication, we ask that you continue to report via the Risk Management email or directly with a risk analyst. You can also submit concerns or questions to Patient Relations for review and response.
