See your best with preventive vision care
Those enrolled in the Asante Health Plan can earn between $50 and $125 toward their HealthEquity account.
When looking at your total health, it’s important to be proactive. One way is to get your vision checked annually. An eye exam could help find underlying health conditions ahead of time.
If you’re enrolled in Asante Health Plan’s vision coverage, you can search for providers through VSP Vision or by calling (800) 877-7195. Your ID number with VSP Vision is your employee number with four zeros in front.
Did you know that you can earn contributions to your HealthEquity account if you or your covered spouse have a preventive vision exam? The value of the incentive will vary based on which plan you are on. Those on the Asante PPO Health Plan will earn a $50 contribution. If you’re enrolled in the Asante Savings Health Plan or the Asante Reimbursement Health Plan you’ll receive a $125 contribution.
There’s no need to provide documentation, the claims will be automatically flagged for the incentive, and the employer contribution will show in your HealthEquity account in four to six weeks.
