Get your flu shot or submit your documentation by Jan. 24
Employees, medical staff, volunteers and students now have until Jan. 24 to get a flu shot or submit their declination form.
If you still haven’t gotten your flu shot or submitted documentation, time is running out. MyChart now offers an in-app/online flu attestation and declination process to make complying with the deadline even easier.
Employees who receive their flu shot from any Asante facility such as an APP clinic, Asante pharmacy or Employee Health, will simply log onto their MyChart account and attest that they have been immunized. Those who received the flu vaccine outside of Asante will use the MyChart form to attest.
