Get your COVID-19 vaccine and earn cash before time runs out

Updated June 25

Don’t forget that the deadline to get your COVID CASH is midnight, June 30. Asante will award $200 (minus required tax withholding) to all eligible employees who complete the vaccination series for COVID-19.

The intent of this COVID CASH program is to increase the overall percentage of the population vaccinated against the virus, especially those working in health care.

The more of us who are vaccinated, the closer we are to achieving herd immunity in our communities — and protecting each other and those we serve. The program ends June 30, so if you haven’t already scheduled your vaccine, you can easily do so in MyChart.

Eligibility criteria:

  • You must be an active full-time, part-time or flexible Asante employee at the time you receive at least the final dose of COVID-19 vaccine.
  • You must be an active employee at the time of the payout.
  • Those enrolled in other incentive programs (besides PEAK) are not eligible.

How do I get the incentive?

Employees are required to complete the “COVID CASH” questionnaire through MyChart, which requests that your vaccination record be shared with Asante Human Resources. Please answer the question at the prompt, then click “Yes” to be added to the list to receive the incentive. If you do not yet have a MyChart account, now would be a great time to get signed up. Visit MyChart on Asante.org and follow the prompts.

NOTE: If you’re just now signing up for MyChart, the assessment question may not appear right away, so check back later. If you don’t have MyChart or cannot see the assessment, you can complete the COVID CASH email questionnaire, which is located in myHR in the Announcements section.

Eligible employees who have already completed a vaccine series and submitted the questionnaire should have seen their bonus starting on their April 14 paycheck. If you have completed all the steps but have not seen your incentive, you may open a case in AskHR for review.

The final day to complete the vaccination series and questionnaire is June 30, 2021 to be eligible for the final incentive payout on the July 9 paycheck.

What if I don’t have a MyChart account?

You can do this by email. Open this document, fill in your information, then copy and paste the contents into an email message. (Do not send as an attachment.) Send the email to the address provided within the document.

What if I received the vaccination outside Asante?

It may take a few days for your health record to update, so please be patient with the process. We will be tracking all currently approved vaccinations: Pfizer, Moderna and Johnson & Johnson.

What if I can’t get the COVID-19 vaccine?

We realize that some employees may not be medically able to receive the vaccination or have strongly held religious convictions that preclude you from receiving it. There is still a way for you to request the incentive.

If you have a religious exemption, email em***************@as****.org with your reason or call (541) 789-2168 or (541) 472-7614 and someone will discuss it with you to verify if you qualify.

If you have a medical/disability reason, email em************@as****.org or call Employee Health at (541) 789-3674.

If approved, you will be required to complete the “COVID-CASH” education module available in the ALEC/HealthStream course catalog.

The questionnaire is not in myChart and the link on myHR has no form.
I completed both Pfizer vaccinations a couple weeks ago.

Sean, someone from HR will reach out to you directly after reviewing your individual situation. It does take some time for the vaccination information to flow over to your EPIC record so that the MyChart questionnaire can be added to your messages.

I got my last vaccine (Moderna) at Bi mart on June 26-2021. There is nothing on mychart to fill out. Not sure what to do.
