The Health Assessment helps you measure your overall health and identify areas of opportunity, so you can get practical and personalized information on how to feel your best. As a bonus, you and your covered spouse can both earn a $25 Amazon gift card for completing it.

The Health Assessment takes about 10 minutes and includes questions on several health topics, including:

• Preventive health. Have you completed the recommended screenings for your age and gender?
• How are your dietary habits and practices impacting your health?
• Are you getting the right amount and type of movement in?
• Emotion and stress. How much is stress affecting your ability to work, live and thrive?
